• You will submit your information and sign up for "$0.00 membership". 
• Once we confirm your patient account, you will be charged for the Consultation Call ($100 one-time charge)
• Estimated completion time (5 minutes)

After signing up for a patient account, you will automatically be redirected to our calendar page to request your Consultation Call with us on the date/time that you select. If you did not request your appointment with XY Men's Health when signing up in Step 1, please use the link below.

• Estimated completion time (2 minutes)

After requesting your Consultation call with us, you will be redirected to our medical history form. Please complete this form prior to your consultation call. If you do not complete this form after scheduling your Consultation call, please use the link below to complete the form.

• Estimated completion time (8 minutes)

After requesting your 15-minute consultation call with us, we will confirm your appointment with you via email. Please log log into this virtual call appointment 5 minutes prior to your appointment from your computer or phone from the meeting link sent to you via email.
