Initial 1 hour Appointment - this includes your first injection, a 6-12 month supply of syringes, alcohol swabs, and a sharp container

The one-time $100 charge for the Consultation Call
Lab Orders - If paying by cash, this is approximately $200 for the initial lab panel and $75 for the quarterly labs that are required every 3 months
Medication Costs - Approximately $50 for a 3-month supply of Testosterone Cypionate. Other medications that may be prescribed are approximately $30 for 3-6 month supply

Patient Payment & Insurance

We do not accept insurance for payment for the Consultation Call or the Membership charges. We do accept ACH and Credit Card payments.
Patients may pay for lab work with their medical insurance (if desired).

Late Cancellation & No-Show Policy

For the Consultation Call:

Late Cancellations (within 24 hours) or "no-shows" will result in a $50 fee in order to reschedule the Consultation Call. The original $99 Consultation Call charge is non-refundable, even if the patient does not choose to reschedule.
A secondary late cancellation (within 24 hours) or "no-show" for the Consultation Call will result in the patient becoming ineligible to reschedule the Consultation Call.

For Patients Paying for a Membership:

For patients paying for a membership, Late Cancellations (within 24 hours) or "no-shows" to any appointment will result in a $50 fee in order to reschedule.

Termination

XY Men's Health reserves the right to terminate a patient's membership for repeated late cancellations or "no shows" as well as for delinquent patient invoices per the signed Patient Agreement form.
